- Pledge Form (Borang ikrar) - UMS/PPPS/01
- Undertaking Dress Code Form (Borang aku janji etika berpakaian)
- Student health examination form (Borang pemeriksaan kesihatan pelajar)
- Surgical authorization letter - UMS/PPP/03
- Undertaking Form (Borang aku janji) - UMS/PPPS/04
Please make sure the copy of the document is certified true and send via google form link before or on 30 September 2022

Completed and bring documents:
1. Passport and copies of passport;
2. Original copies of Bachelor's degree and transcript;
3. IELTS/TOEFL/ISFP/MUET/PEP certificate;
4. The original letter of offer for education funding / scholarships /
financial assistance (if any);
5. Completed form (UMS/PPPS/01 - 04) and
6. Proof of payment (if you made the payment via FPX) or Jompay
*Candidates who cannot attend face-to-face, can still register online until September 18, 2022 and submit document via google form (Step 4).
